When developing innovative therapeutics, recruiting adequate numbers of suitable patients is undoubtedly a major cornerstone in a successful clinical trial. It is estimated that less than 10% of all clinical trials are completed on time and patient recruitment is considered to be the rate-limiting step. Inefficient adaptations by study sponsors, unrealistic and complex protocols as well as misinformed assessments of study sites are among the primary hurdles that hinder patient recruitment and retention. The prevalence and chronic nature of metabolic diseases has led investigators to explore solutions to the aforementioned problems as well as challenges that are unique to diabetes and obesity. Cultural differences and the demanding nature of long-term trials make patient retention an increased challenge in chronic diseases. Given the limited therapeutic targets for diabetes, there is also a shortage of absolutely novel antidiabetic drugs that can motivate patients to enroll on their own merit.

It is widely accepted that the traditional methods of patient recruitment and retention for diabetes trials are not sustainable in diverse and aging populations. The limited therapeutic targets for diabetes yields very few first-in-class drugs and this slows patient recruitment and keeps it competitive among trial sponsors. New outlooks and collaborations have to be explored to motivate patients to enroll and stay in the study.
