HIPAA`s existing privacy and security regulations do not include a definition of unsecured protected health information!!   The HITECH Act therefore required the secretary of Health and Human Services (HHS) to issue guidance specifying the technologies and methodologies that render protected health information "unusable, unreadable or indecipherable" to unauthorized persons.

The Office for Civil Rights (OCR) is significantly raising the ante for HIPAA Security compliance.  Incredibly, arguably the majority of Covered Entities (CEs) appearing on the OCR Data Breach Notification website or "Wall of Shame" ( http://ow.ly/42eW0 ) would not be there if proper encryption solutions been deployed.

Secured electronic Protected Health Information (ePHI) essentially means the ePHI has been encrypted. (It also may mean it has been rendered inaccessible = destroyed).
